We understand how tedious it can be to fill out paperwork in the waiting room, so we try to expedite the process by conveniently having all the forms you may need right here on this page.

If this is your child’s first visit, our pediatric dentist, Dr. Andrea Salazar Dinh, will first review your child’s dental and medical history before thoroughly examining your child’s teeth. From there, she’ll go over any issues and patiently discuss all options for treatment. We then typically proceed with pediatric dental services you need at a time that is most convenient for you.
